Employee was "overwhelmed" by excessively complex task: coroner

Stress that stemmed from a toxic workplace culture and sub-optimal return-to-work management was the primary cause of an employee's death, a coronial investigation has found.

Victoria's Deputy State Coroner Paresa Antoniadis Spanos found the Ballarat Base Hospital accountant, Karla Lee Jordan, died by suicide on 29 August 2019, two days after she was given an "excessively complex task" that overwhelmed her.

The Coroner noted "significant deficiencies in the workplace culture" was the employee's "primary identifiable suicide stressor"...
